The Safety Reporting System has transitioned to using PurpleID (Okta Verify). There is an intermediate step requiring an Okta Verify token to verify your identity. Once this is completed, you will continue to the reporting portal as previously accomplished.

As part of our overall safety goals, FedEx Express has established a system for our Flight Crew, Maintenance, and Dispatchers to voluntarily identify and report safety and operational concerns. The collected information is reviewed and analyzed to facilitate early detection and improved awareness of operational deficiencies and adverse trends. The information specified in these reports are used to identify the root causes and determine appropriate remedial actions which are then monitored for effectiveness. This process promotes collaboration between work groups and management for the early identification of hazards and to maintain a proactive approach regarding safety concerns and corrective action recommendations.

FedEx Express, in cooperation with our pilot group and the FAA, has established an Aviation Safety Action Program (ASAP) for Flight Crew, Maintenance, and Dispatchers. The FedEx Express ASAP is a voluntary, self-reporting program designed to identify and reduce possible safety concerns. ASAP uses employee input to identify significant safety concerns and issues, operational deficiencies, non-compliance with regulations; deviations from company policies and procedures, and unusual events. In partnership with all relevant departments and the FAA, each report is investigated and corrective actions determined based on a non-punitive approach to safety.

FedEx Express has developed a system for the submission of incident and event reports by Flight Crew and Maintenance to identify safety and operational deficiencies. These reports will be investigated and analyzed to identify hazards, determine appropriate safety and/or operational improvements, monitor the effectiveness of corrective actions, and proactively promote employee awareness of potential hazards.

As part of our overall safety goals, and to comply with FAA Safety Management System (SMS) regulations, FedEx Express accomplishes Safety Risk Management (SRM) by using procedures developed to understand the critical characteristics of its operational systems and environment. We must apply SRM to our air carrier functions under any of the following conditions:

  1. Implementation of new systems
  2. Revision of existing systems
  3. Development of operational procedures
  4. Identification of hazards or ineffective risk controls through the Safety Assurance process
