The HA Safety / Security Reporting Program provides a means for all employees to identify and report safety, security, and operational concerns as well as meet mandatory reporting requirements for incidents, injuries and damage to equipment or facilities. The information supplied by employee reports is reviewed and analyzed to facilitate early detection and improved awareness of operational deficiencies and adverse trends. This is accomplished by identifying the root causes and determining appropriate remedial actions which are then monitored for effectiveness. This process promotes collaboration between employee work groups and management for the early identification of hazards and to maintain a proactive approach to a safe and secure work environment.

Incident reports are submitted electronically through this data collection program. Refer to manuals specific to your employee group for more detailed incident reporting requirements. Managers may also be contacted for information regarding incident reporting for their respective employee group.

Incident reports should be initiated as soon as possible. Narrative blocks within the form should include complete and concise information.

HA in cooperation with our employee labor organizations (and the FAA for the ASAP Program), has established two confidential, voluntary, self-reporting programs to identify and reduce possible safety concerns.

  • ASAP - The Aviation Safety Action Program is available for Flight Crews (PSAP), Dispatchers (DSAP) and Maintenance (MSAP).
  • GSIP - The Ground Safety Improvement Program is available to Customer Service, GSE/Facilities and Stores employees.

These programs use employee input to identify significant safety concerns and issues; operational deficiencies; non-compliance with regulations; deviations from company policies and procedures; and unusual events. Each report is investigated and corrective actions determined based on a non-disciplinary approach to safety
