United Aviate Academy has established this system for our staff and students to voluntarily identify and report safety and operational concerns as part of our overall Safety Management System (SMS).
This confidential safety reporting process promotes collaboration between employees, students, and management to maintain a proactive non-punitive approach regarding safety concerns and corrective action recommendations. Submitted information is collected, reviewed, and analyzed to facilitate early detection and improved awareness of operational deficiencies and adverse trends.
The information gathered from reports is used to identify hazards, discover root causes, and to develop effective mitigations designed to prevent reoccurrence and reduce operational risk.
Safety Reports should be initiated as soon as possible. Narrative blocks within the form should include complete and concise information. Refer to the UAA Safety Management Manual and specific department manuals for more detailed safety reporting requirements.
