Safety Reporting System

Southern Air’s Safety Reporting System (SRS) is designed to underpin a just and safe culture. It allows all employees to report and receive feedback regarding hazards, operational concerns, incidents, injuries, and accidents. Southern Air reviews and analyzes the collected information to facilitate early detection and improved awareness of operational deficiencies and adverse trends as well as react to emergent situations. The information is used to identify the root causes and determine appropriate remedial actions which are placed in action, then monitored for effectiveness. The process promotes collaboration between employee work groups and management for the early identification of hazards, proactive risk management and is purposed to develop a generative Safety culture.

Southern Air, in cooperation with the FAA, has established an Aviation Safety Action Program (ASAP) for Flight Crew, Maintenance, Dispatch, and Loadmaster employees. Southern Air ASAP is a voluntary, self-reporting program designed to identify and reduce possible safety concerns. ASAP uses employee input to identify significant safety concerns and issues; operational deficiencies; non-compliance with regulations; deviations from company policies and procedures; and unusual events. In partnership with the relevant department and the FAA, each report is investigated and corrective actions determined based on a non-disciplinary approach to flight safety.

The purpose of Incident Reporting is to determine root cause(s) of the incident to prevent its recurrence - not to find fault or lay blame. All FAA reportable incidents are investigated by the Department Manager involved. All events involving aircraft activity on the ground or in-flight, whether regarding SAI Maintenance, Crew/Aircraft Operations, System Operations Control, External Service Providers, Regulatory Authorities, or Air Traffic Control, require the submission of an Incident Irregularity Report (IIR). Any Company employee who witnesses and/or is associated with a reportable event is required to submit an IIR, when requested by their supervisor.

As part of the Southern Air Safety Management System (SMS), any Southern Air employee may submit a Hazard Report. Southern Air provides all employees various means, including WBAT, to report unsafe conditions and failures in operating processes and procedures to management. The hazard identification program is non-punitive and identifies specific hazards and mitigates their effects to improve flight and ground safety. Employees may broach safety concerns and operational deficiencies, identify non-compliance with regulations, show deviations from company policies and procedures, and relate possible cause – effect of problems noted.

Whenever a Southern Air employee is injured on the job or anyone is injured on Company property, the individual (if able) and the immediate supervisor or station manager shall complete a Personal Injury Report form describing the accident/incident. The information specified in the Injury Report will be used to identify the root causes and determine appropriate remedial action to help to mitigate the possibility of the injury occurring again. Injuries requiring medical assistance beyond first aid are considered recordable as OSHA events and are forwarded to HR. All suspected or possible after-effect type events should be submitted. Safety will maintain records of all events submitted.
