FLIGHT PROGRAM SAFETY REPORTING SYSTEM

The Flight Program utilizes the Web-Based Application Tool (WBAT) for online safety reporting. WBAT is a secure, web-based software system that supports the submission, management and analysis of confidential employee reports via the web or mobile device. It provides a channel for both the mandatory and voluntary reporting of aviation occurrences or hazards relevant to our organization’s aviation activities, while protecting the reporter’s identity. These reports are used to help identify areas of risk to our flight operations in a proactive manner and implement corrective actions in order to reduce the potential of an incident or accident. Additionally, WBAT is used to document change management, safety risk assessments, and internal evaluations in support of the Flight Program Safety Management System (SMS).

There are two types of reports that can be submitted through WBAT:

ASAP Report

ASAP is applicable to pilots, mission specialists, dispatchers, and maintenance participants. Through ASAP, participants can voluntarily report safety-related events or concerns to management and to the FAA Certificate Management Unit (CMU) for resolution. An ASAP report should be submitted for:

  • Any safety information that may be critical to identifying potential precursors to accidents

Safety Report

A safety report must be submitted for:

  • Any condition, situation, procedure, behavior, or policy identified as a real or potential hazard affecting Flight Program Operations
  • Any event that occurs in flight or on the ground, other than an aircraft accident or incident as defined in 49 CFR Part 830, which affects or could affect the safety of an FAA-operated aircraft or aircrew member
  • Specific to flight crews, any use of the abnormal or emergency checklist
