Safety Reporting

Grant Aviation utilizes the WBAT online database system as the ‘online reporting system’ for all employees to identify and report operational safety concerns. Reported information is reviewed, analyzed and tracked to facilitate early detection and improved awareness of operational deficiencies and adverse trends. Information specified in employee reports is used in combination with safety risk management and safety assurance processes to identify causes and determine appropriate mitigating or corrective actions which are then monitored for effectiveness. This process enables collaboration between employee work groups and management for early detection of hazards and to maintain a proactive approach regarding operational safety concerns and corrective action recommendations.

Reports submitted through the online reporting system shall be non-punitive and confidential.

Types of reports to be submitted through the online reporting system:

  • Safety concern
  • ASAP
  • Incident
  • Near Miss
  • Process Improvements
  • Self-Disclosure or observations of non-compliance with policies or procedures

Grant Aviation, its employees and the FAA, have implemented an Aviation Safety Action Program (ASAP). This program is a voluntary, self-reporting program designed to identify and reduce possible operational safety concerns. ASAP uses employee input to identify operational safety concerns and issues; operational deficiencies; non-compliance with regulations; deviations from company policies and procedures; and unusual events. Each report is reviewed and associated mitigating or corrective actions determined based on a non-disciplinary approach to flight safety.

This ASAP program applies to all Pilots, Mechanics, and Dispatch agents; and only to events that occur while acting in that capacity.

A report will not be accepted into ASAP when the event involves any of the following:

  • Intentional noncompliance with the Code of Federal Regulations (CFR’s)
  • Intentional disregard for safety
  • Criminal activity
  • Substance abuse, controlled substances, alcohol use
  • Intentional falsification of any documentation
