Employee Safety Reporting

As part of our overall safety goals, Endeavor Air has established this Safety Reporting System (WBAT) for our Cabin, Dispatch, Flight Crew, and Maintenance employees to submit reports to the Company to identify and report safety and operational concerns as well as fatiguing events. The collected information is reviewed and analyzed to facilitate early detection and improved awareness of operational deficiencies and adverse trends. The information specified in employee reports are used to identify the root causes and determine appropriate remedial actions which are then monitored for effectiveness. This process promotes collaboration between employee work groups and management for the early identification of hazards and to maintain a proactive approach regarding safety concerns and corrective action recommendations. Endeavor Air, in cooperation with our employee labor organizations and the FAA, has established an Aviation Safety Action Program (ASAP) for Cabin, Dispatch, Flight Crew, and Maintenance employees. The Endeavor Air ASAP is a voluntary, self-reporting program designed to identify and reduce possible flight safety concerns. ASAP uses employee input to identify significant safety concerns and issues, operational deficiencies, non-compliance with regulations, deviations from company policies and procedures, and unusual events. In partnership with all of the relevant departments, labor organizations, and the FAA, each report is investigated and corrective actions determined based on a non-disciplinary approach to flight safety, as established by each program's MOU.

Endeavor Air has developed a system for the submission of incident and event reports by our Cabin, Dispatch, Flight Crew, and Maintenance employees to identify safety and operational deficiencies within their respective departments. These reports will be investigated and analyzed to identify hazards; determine appropriate safety and/or operational improvements; monitor the effectiveness of corrective actions, and proactively promote employee awareness of potential problems.

Endeavor Air, in compliance with our Fatigue Risk Management Plan, has established this web-based submittal and database system for our Cabin and Flight Crew employees. Cabin and Flight Crew employees are required to submit a Fatigue report when they call in fatigued for duty and that fatigue reporting is to be accomplished through this system. The company also encourages reports to be submitted for situations that are fatiguing in nature, but did not result in a fatigue call to the company; these fatiguing situation reports can be submitted here as well. The Fatigue Reports on fatigue calls or on fatiguing situations will be used to mitigate and manage the effects of flight crewmember fatigue and improve flight crewmember alertness.
